I think the client often writes down the words, but it's the artist's reputation on the line so some fact checking is definitely in order. I work in print (as the customer of the printed material) too, and although we ask them to check, the responsibility ultimately lies on us to get it right, as far as what is desired. That's the point of getting a proof, after all. As far as tattoos, it's your (collective your) body, you're going to own it, so you should take the responsibility for it being as you want it.
